Step-1: You will receive an email from the portal of verification - Contractor Compliance.

• Activate your account in Contractor Compliance. Please refer to the instructions provided in this document: Contractor Compliance Process.pdf

• Remember to save your login information (email and password) for future reference. Future access to the Contractor Compliance Portal can be found here: https://www.contractorcompliance.io/ You can use this link to update any expired or outdated documents as needed.

Step-2: Complete the Drug and Alcohol Consortium Consent Form (CC) and pass the drug test

• Test Cost: $70

• Contact consortium (Test On Time) agent by text. 

  • Julian Preston, CEO & Compliance Officer, via text at 310-310-1088. 

  • Only communicate via text; calls or WhatsApp messages will not be accepted.

  • In your text, provide a brief introduction of yourself and indicate that you require Drug and Alcohol Testing for LXD Texas LLC, including your address.

  • The Consortium will send you a list of the nearest providers for the test and will request payment. 

  • After making the payment, you will receive a barcode that you need to bring to the selected lab to take the test. Proceed to the location that is most convenient for you.

• Once we have received the laboratory results, we will send you the "Drug Test Consent Form" to be completed via email through Jotform.

*you may be subject to a random drug test at any time (this cost will not be covered by the company)*

Step-3: Finish the “SafeRide Driver Education Course”

• Cost $25

• https://everdriven.talentlms.com/index

• Click "Sign Up" and create your profile.

• After you log in, click on "Course Catalogue"

• Complete the "Introduction to SafeRide Driver Education Course" 

• Complete the "Course for all other vehicles" (unless you have a wheelchair-accessible vehicle)

• The "Course for wheelchair-accessible vehicles" does not need to be completed unless you have a wheelchair-accessible vehicle. 

• Once you finish the course, upload the certificate to the Contractor Compliance Portal. 

Step-4: Undergo fingerprints.

• Apply here: https://psp.azdps.gov/fingerprintClearance/formTriage 

• Click that you require an IVP Card

• Create an account for the Finger Print Clearance Card

• Verify your email and log in to your new account 

• Click on "Fingerprint Clearance Card"

• Click on "Apply for a Card / Request Replacement"

• Continue with the application for a new IVP Card

• Do not apply for a different card to the IVP Card. 

• Please send us the new card via WhatsApp or upload it directly to the Contractor Compliance Portal
  
  

Step-5: Vehicle inspection

• After completing all the preceding steps, LXD Texas LLC will request a video and pictures of your vehicle for a virtual inspection.

• Here are the instructions to submit the pictures and information 

  • For virtual inspections, please send the following via WhatsApp to our manager: 

    1. Four (4) distance pictures of your vehicle, one picture per side.

    2. A picture of your vehicle's millage.

    3. Please upload a picture of your state inspection sticker. 

    4. A picture of your license plate.

    5. A picture of your vehicle’s VIN.

    6. A picture of every seat belt.

    7. A video of your child lock working properly for the windows and doors.

    8. A video of the headlights, blinkers, and rear lights working properly. 

• Furthermore, please ensure that your vehicle is equipped with a fire extinguisher, a first aid kit, and a web cutter to comply with safety regulations.

Step-6: Safety Equipment and Training

• First Aid and CPR Certification ($37)

• Defensive Driving ($25)

• Get a DOT Physical Exam done. Recommended locations: Concentra Urgent Care clinic

• Send these documents via WhatsApp or upload them directly to the Contractor Compliance Portal.

Step-7: Independent Contractor Contract with LXD Texas LLC, W-9 Form, and Direct Deposit.

• Please sign your contract as an Independent Contractor with LXD Texas LLC

  • You will receive an email notification from Jotform containing the links to sign and complete the document electronically.

• Complete the W-9 form.

  • This form can be completed here: https://form.jotform.com/231655234409152 

• Complete the Direct Deposit form with your bank information to receive your payments. 

  • This form can be completed here: https://form.jotform.com/232405876998070 

Step-8: Entrance Fee Receipts

• Please fill out the Entrance Fee Receipts form (Drug Test, PASS Basic, and Fingerprints) to receive your refund after completing twelve work weeks with LXD Texas LLC.

Step-9: App installation, and activation

• Once the company has verified all your information, you will receive your ID number (MDD and Pin)

• Following that, you will undergo training on how to install and utilize the application (EverDriven App) for your work. 

• After completing the training, we highly recommend carefully reading the manual before entering the field.

Step-10: Daily Protocol Training, Decal, Vest, and Badge.

• ​Please contact our Director of Operations, who will provide you with the necessary guidance to provide excellent service.

• In addition, you will receive your identification elements such as the sticker for your car, the company vest, and your badge.

Paso-5: Inspecciones del vehículo.

Paso-6: Equipo de seguridad y capacitación

Paso-9: Instalación y activación de la aplicación.

• Una vez que la empresa haya verificado toda su información, recibirá su número de identificación (MDD y un Pin)

• Después de completar el entrenamiento, recomendamos leer cuidadosamente el manual antes de ingresar al campo y realizar su primer servicio.

Paso-10: Capacitación protocolo diario, calcomanía, chaleco y credencial.

• Por favor contactar a nuestra Directora de Operaciones, quien le brindará la orientación necesaria para prestar un excelente servicio. 

• Igualmente, le entregará los elementos de identificación como la calcomanía (o sticker) para su carro, el chaleco y la credencial.

• Visit the Office from 10 am-4 pm, Monday-Friday, located in downtown Santa Cruz at 841 Cedar Street. Walk-ins are welcome. 

• First, please take the Request for Live Scan Form, your non-expired government-issued ID, and some form of payment.
  
  

Step-5: Vehicle inspection

  • •  Once your vehicle is inspected and receives a “PASS”, simply take a photo of the filled-out form and upload it at http://contractorcompliance.io/ to complete the signup process

Step-6: Tuberculosis Test

• TB tests can done at both General Health Clinics, in Santa Cruz at 1080 Emeline Ave (Bldg D), and in Watsonville at 9 Crestview Dr. 

  • • TB tests are done on Monday, Tuesday, Wednesday, and most Fridays between 1:00 PM - 4:00 PM. 

    • Patients need to return to the clinic 48 hours after the TB test to have the test read. 

• TB tests can also be done at your local CVS.

Step-6: Bus Driver Training and Code of Conduct

Step-1: You will receive an email from the portal of verification - Contractor Compliance.

• Activate your account in Contractor Compliance. Please refer to the instructions provided in this document: Contractor Compliance Process.pdf

• Remember to save your login information (email and password) for future reference. Future access to the Contractor Compliance Portal can be found here: https://www.contractorcompliance.io/ You can use this link to update any expired or outdated documents as needed.

Step-2: Complete the Drug and Alcohol Consortium Consent Form (CC) and pass the drug test

• Test Cost: $70

• Contact consortium (Test On Time) agent by text. 

  • Julian Preston, CEO & Compliance Officer, via text at 310-310-1088. 

  • Only communicate via text; calls or WhatsApp messages will not be accepted.

  • In your text, provide a brief introduction of yourself and indicate that you require Drug and Alcohol Testing for LXD Texas LLC, including your address.

  • The Consortium will send you a list of the nearest providers for the test and will request payment. 

  • After making the payment, you will receive a barcode that you need to bring to the selected lab to take the test. Proceed to the location that is most convenient for you.

• Once we have received the laboratory results, we will send you the "Drug Test Consent Form" to be completed via email through Jotform.

Step-3: Finish the “SafeRide Driver Education Course”

Step-4: Undergo fingerprints.

• Cost $25.25

• Enter Code: 1KG6XN

• Once Registration is complete, go to an IdentoGO facility for fingerprinting (bring ID)

• After fingerprinting is complete, retain the UEID number.

• Results will be emailed. *The link to results only works ONCE. Make sure to open link on a computer AND save the results and upload them to the Contractor Compliance Portal to "Act 114".

Step-5: Vehicle inspection

• After completing all the preceding steps, LXD Texas LLC will schedule an in-person appointment to inspect your vehicle.

• Furthermore, please ensure that your vehicle is equipped with a fire extinguisher, a first aid kit, and a web cutter to comply with safety regulations.

Step-6: Obtain a DOT Physical Exam Results (Good for 2 years)

• Follow these steps during the registration process; please enter the information exactly as it appears on your government-issued ID

3. Choose your Name/Method of Contact and provide the required information. Proceed to the next step.

4. Enter TX921220Z for the Distrito ORI.

5. Enter your agency identifiers as requested and proceed.

6. Provide your citizenship information.

7. Answer the questions as applicable. 

• For the question regarding an Authorization Code (Coupon Code) select 'No'.

8. Enter your alias and personal information, as applicable. Proceed to the next step.

9. Fill in the required information for your mailing address. Proceed to the next step.

10. Select the required documents you will bring to your fingerprinting appointment and answer the applicable questions. Proceed to the next step.

11. Enter your zip code for the location and proceed.

12. Choose your appointment date and time. Submit your selection.

Step-5: Vehicle inspection

• After completing all the preceding steps, LXD Texas LLC will request a video and pictures of your vehicle for a virtual inspection.

• Here are the instructions to submit the pictures and information 

  • For virtual inspections, please send the following via WhatsApp to our manager: 

    1. Four (4) distance pictures of your vehicle, one picture per side.

    2. A picture of your vehicle's millage.

    3. Please upload a picture of your state inspection sticker. 

    4. A picture of your license plate.

    5. A picture of your vehicle’s VIN.

    6. A picture of every seat belt.

    7. A video of your child lock working properly for the windows and doors.

    8. A video of the headlights, blinkers, and rear lights working properly. 

• Furthermore, please ensure that your vehicle is equipped with a fire extinguisher, a first aid kit, and a web cutter to comply with safety regulations.

Step-6: Independent Contractor Contract with LXD Texas LLC, W-9 Form, and Direct Deposit.

• Please sign your contract as an Independent Contractor with LXD Texas LLC

  • You will receive an email notification from Jotform containing the links to sign and complete the document electronically.

• Complete the W-9 form.

Step-7: Entrance Fee Receipts

Step-8: App installation, and activation

• Once the company has verified all your information, you will receive your ID number (MDD and Pin)

• Following that, you will undergo training on how to install and utilize the application (EverDriven App) for your work.

Step-9: Daily Protocol Training, Decal, Vest, and Badge.

• ​Please contact our Director of Operations, who will provide you with the necessary guidance to provide excellent service.

• Remember to save your login information (email and password) for future reference. Future access to the Contractor Compliance Portal can be found here: https://www.contractorcompliance.io/ You can use this link to update any expired or outdated documents as needed.

Step-2: Complete the Drug and Alcohol Consortium Consent Form (CC) and pass the drug test

• Test Cost: $70

• Contact consortium (Test On Time) agent by text. 

  • Julian Preston, CEO & Compliance Officer, via text at 310-310-1088. 

  • Only communicate via text; calls or WhatsApp messages will not be accepted.

  • In your text, provide a brief introduction of yourself and indicate that you require Drug and Alcohol Testing for LXD Texas LLC, including your address.

  • The Consortium will send you a list of the nearest providers for the test and will request payment. 

  • After making the payment, you will receive a barcode that you need to bring to the selected lab to take the test. Proceed to the location that is most convenient for you.

• Once we have received the laboratory results, we will send you the "Drug Test Consent Form" to be completed via email through Jotform.

  • Click the Abuse and Neglect course

  • Click the link for mandatory reporting community response for youth and families

  • Complete lesson 1-5, then take the assessment (70% or higher is needed to pass)

  • Once passed please download the certificate and email it to admin@lxdtexasllc.com

Step-3: Finish the “SafeRide Driver Education Course”

• Cost $25

• After you log in, click on "Course Catalogue"

• Complete the "Introduction to SafeRide Driver Education Course" 

• Complete the "Course for all other vehicles" (unless you have a wheelchair-accessible vehicle)

• The "Course for wheelchair-accessible vehicles" does not need to be completed unless you have a wheelchair-accessible vehicle. 

• Once you finish the course, upload the certificate to the Contractor Compliance Portal. 

Step-4: Undergo fingerprints.

•  Cost $ 39.05 

• Pre-register for your fingerprinting appointment at

• Follow these steps during the registration process; please enter the information exactly as it appears on your government-issued ID

1. Enter Service Code 11FJ5N and click "Go"

2. Select "Schedule or Manage Appointment"

3. Choose your Name/Method of Contact and provide the required information. Proceed to the next step.

4. Enter TX922565Z  for the Distrito ORI.

5. Enter your agency identifiers as requested and proceed.

6. Provide your citizenship information.

7. Answer the questions as applicable. 

• For the question regarding an Authorization Code (Coupon Code) select 'No'.

8. Enter your alias and personal information, as applicable. Proceed to the next step.

9. Fill in the required information for your mailing address. Proceed to the next step.

10. Select the required documents you will bring to your fingerprinting appointment and answer the applicable questions. Proceed to the next step.

11. Enter your zip code for the location and proceed.

12. Choose your appointment date and time. Submit your selection.

Step-5: Vehicle inspection

• After completing all the preceding steps, LXD Texas LLC will either schedule an in-person appointment to inspect your vehicle.

• Furthermore, please ensure that your vehicle is equipped with a fire extinguisher, a first aid kit, and a web cutter to comply with safety regulations.

Step-6: Independent Contractor Contract with LXD Texas LLC, W-9 Form, and Direct Deposit.

• Please sign your contract as an Independent Contractor with LXD Texas LLC

  • You will receive an email notification from Jotform containing the links to sign and complete the document electronically.

• Complete the W-9 form.

Step-7: Entrance Fee Receipts

• Please fill out the Entrance Fee Receipts form (Drug Test, PASS Basic, and Fingerprints) to receive your refund after completing twelve work weeks with LXD Texas LLC.

Step-8: App installation, and activation

• Once the company has verified all your information, you will receive your ID number (MDD and Pin)

• Following that, you will undergo training on how to install and utilize the application (EverDriven App) for your work. 

• After completing the training, we highly recommend carefully reading the manual before entering the field.

Step-9: Daily Protocol Training, Decal, Vest, and Badge.

• ​Please contact our Director of Operations, who will provide you with the necessary guidance to provide excellent service.

• In addition, you will receive your identification elements such as the sticker for your car, the company vest, and your badge.

Paso-1: Contractor Compliance

Paso-2: Completar el formulario de Consentimiento del Consorcio de Drogas y Alcohol (CC) y aprobar el examen de drogas.

  • Solo se acepta la comunicación por texto; no se aceptan llamadas ni mensajes de WhatsApp.

Paso-5: Inspecciones del vehículo.

• Después de completar todos los pasos anteriores, LXD Texas LLC programará una cita en persona para inspeccionar su vehículo.

• Además, por favor asegurarse de que su vehículo esté equipado con un extintor de incendios, un botiquín de primeros auxilios y un cortador de tela para cumplir con las normas de seguridad.

Paso-6: Contrato de Contratista Independiente con LXD Texas LLC, Formulario W-9 y Forma para Depósito Directo.

• Por favor, firme su contrato como Contratista Independiente con LXD Texas LLC.

Paso-8: Instalación y activación de la aplicación.

• Una vez que la empresa haya verificado toda su información, recibirá su número de identificación (MDD y un Pin)

• A continuación, recibirá entrenamiento de cómo instalar y utilizar la aplicación (EverDriven App) para realizar su trabajo. 

• Después de completar el entrenamiento, recomendamos leer cuidadosamente el manual antes de ingresar al campo y realizar su primer servicio.

Paso-9: Capacitación protocolo diario, calcomanía, chaleco y credencial.

• Por favor contactar a nuestra Directora de Operaciones, quien le brindará la orientación necesaria para prestar un excelente servicio. 

• Igualmente, le entregará los elementos de identificación como la calcomanía (o sticker) para su carro, el chaleco y la credencial.
